Physical Examination

A current physical examination form must be reviewed by the school nurse before a student-athlete is cleared to participate in practice or tryouts.  Physicals are valid for 1 year from the date of the exam. Click here to download the Physical Examination Form.  

Health History Update

If you have a valid Physical examination on file with the school nurse, then a Health History Update Questionnaire is required.

Please Note: These medical clearance forms can be uploaded into the online registration (Step #4) OR emailed directly to mkakolewski@mahwah.k12.nj.us.
